This site collects contact details and preferences only. It does not collect, store, or process any health information.
What we collect
Information you enter into the clinician interest form: name, email, optional phone number, credentials, states of licensure, practice type, specialty, interest level, and any message you write. Where you contact us by email, whatever you choose to include. Standard server logs and privacy-respecting usage analytics.
What we never collect here
No health information of any kind. There are no symptom forms, no health questionnaires, and no document uploads on this site. Please do not send patient information to the Institute; if you do, we will delete it and tell you that we have.
How we use it
To reply to you, to assess fit for certification, and to administer the credential registry and the practitioner directory. Form submissions are mirrored to our marketing and CRM system so that our team can follow up. We do not sell personal information.
Directory and registry data
Information published in the practitioner directory and the public credential register is professional information published with the clinician's agreement: name, credentials, credential ID and status, practice details, published pricing, and booking link. Clicking through to a practice is logged without any personal data about the visitor.
Retention, access, and contact
We keep enquiry records for as long as needed to administer the relationship, and then delete them. To access, correct, or delete your information, email privacy@healthopiainstitute.com.
